Home Care Package (HCP) is a government - funded initiative designed to empower older Australians to continue living safely and comfortably in their own homes for as long as possible. These packages offer tailored support for individuals with varying and more complex care needs, providing a comprehensive alternative to moving into residential aged care.

Your journey begins by reaching out to My Aged Care (1800 200 422) or their website:https://www.myagedcare.gov.au to register and discuss your care needs. By doing this you will trigger an Aged Care Assessment Team (ACAT) review.

After your assessment, you will receive a letter from the ACAT detailing their recommended care level. Following this, you will be placed in a national priority queue, and the letter will provide an approximate timeframe for your package assignment.

Once approved, a confirmation letter will be sent to you. This letter will contain your assigned Home Care Package level (1-4) and a code to enable you to access services.

When your package is assigned, contact our friendly team. We will then partner with you to manage your package funds and coordinate a personalised care plan that perfectly suits your needs.
